Spaghetti Sauce and Ground Beef, what did I do wrong?

crappy piece of meat. was it beef, or ground chuck? we always use ground chuck for burgers and meat . better flavor, for some reason. i use lawry salt only. i never add onions to spaghetti sauce. i also never add tomatoes. seems to work well. i mix prego with a good brand/quality sauce. prego makes sauces thicker, but straight prego upsets my stomach.
